  • Have a confirmed or suspected G6PD deficiency (Glucose-6-Phosphate Dehydrogenase deficiency) — in individuals with this genetic condition, methylene blue can cause hemolytic anemia, a potentially serious breakdown of red blood cells.
  • Have a known allergy or hypersensitivity to methylene blue or any thiazine dye.
  • Are currently taking serotonergic medications (see interaction section below).
  • Are under the age of 18 without explicit guidance from a pediatric healthcare provider. 

Potential Side Effects 

When used responsibly at low to moderate doses, methylene blue is generally well-tolerated by most healthy adults. However, some users may experience side effects, which can include: 

Common & generally harmless: 

  • Blue or green discoloration of urine and stool — this is expected, normal, and temporary.
  • Mild skin or mucous membrane discoloration at higher doses.
  • Mild nausea, particularly if taken on an empty stomach.
  • Headache, especially when starting supplementation. 

Less common, dose-dependent: 

  • Dizziness or lightheadedness.
  • Elevated heart rate or palpitations.
  • Feeling of restlessness or increased agitation.
  • Sweating or flushing. 

At high doses (not recommended for general supplementation): 

  • Methemoglobinemia — a condition where the blood's oxygen-carrying capacity is impaired. This is primarily a risk at very high doses and is paradoxically the condition low doses are used medically to treat.
  • Confusion, chest pain, or shortness of breath — seek immediate medical attention if these occur.

Drug & Supplement Interactions 

This is one of the most important sections to read. Methylene blue has clinically significant interactions with several categories of medications and supplements: 

⚠️ Serotonergic Medications — Serious Interaction Risk 

Methylene blue inhibits monoamine oxidase (MAO-A), which means it can elevate serotonin levels in the brain. When combined with serotonergic drugs, this can lead to serotonin syndrome — a potentially life-threatening condition. Avoid combining methylene blue with: 

  • SSRIs (e.g., fluoxetine, sertraline, escitalopram)
  • SNRIs (e.g., venlafaxine, duloxetine)
  • MAOIs (e.g., phenelzine, selegiline)
  • Tricyclic antidepressants
  • Tramadol, meperidine, or linezolid
  • St. John's Wort (a commonly used herbal supplement)
  • 5-HTP or high-dose tryptophan supplements 

⚠️ Medications Processed by CYP Enzymes 

Methylene blue may affect liver enzymes (particularly CYP2C19 and CYP1A2) that are responsible for metabolizing many common drugs. This may alter the blood levels of those medications. Consult your doctor if you take any prescription medication regularly. 

ℹ️ Other Supplements to Use with Caution 

  • NMN / NR (NAD+ precursors): No established dangerous interaction, but both affect mitochondrial and redox pathways; monitor your response if combining.
  • Alpha-lipoic acid: May interact with the redox activity of methylene blue; use cautiously and at separate times.
  • Stimulants or high-dose caffeine: May compound cardiovascular effects such as elevated heart rate.
  • Blood thinners (e.g., warfarin): Limited data, but caution is advised; monitor with your physician. 

Dosage Responsibility

More is not better with methylene blue — lower doses in the range of 0.5–4 mg/kg are associated with positive effects in research settings, while higher doses can produce the opposite of the desired outcomes.
